Our responsibilities
We are required by law to protect the privacy and security of your health information. This notice applies to all care at Meaningful Medicine Clinic, including acupuncture, integrative medicine, and primary care, whether in person or by telehealth.
We must follow the duties and privacy practices described in this notice and give you a copy of it.
We will not use or share your information other than as described here unless you tell us we can in writing.
We will let you know promptly if a breach occurs that may have compromised the privacy or security of your information.
Our clinicians and staff share information with each other as needed to coordinate your care within the clinic.
How we typically use and share your information
Treatment. We use your information to provide your care and share it with other professionals treating you. Example: With your consent, we send a treatment summary to your primary care doctor, fertility clinic, or specialist.
Payment. We use and share your information to bill and get paid. Example: We provide you with a superbill containing diagnosis and treatment codes so you can seek reimbursement from your health plan, or we share the information with your plan if we bill it directly.
Health care operations. We use your information to run our clinic, improve care, and contact you when needed. Example: We use your information to schedule appointments, send reminders, and review the quality of our care.
Business associates. Some services are provided by outside companies, such as our electronic health record and scheduling system, telehealth platform, and billing services. They must sign agreements to protect your information.
Other ways we may use or share your information
The law allows or requires us to share information in certain situations, usually only after specific conditions are met. These include:
Public health and safety: reporting disease, adverse reactions to products, suspected abuse or neglect, or preventing a serious threat to someone's health or safety.
Health oversight: responding to audits, investigations, and licensing activities by agencies such as the Oregon Medical Board.
Legal requirements: when state or federal law requires it, including to the U.S. Department of Health and Human Services to show we comply with privacy law.
Lawsuits and legal actions: in response to a court or administrative order, or a subpoena that meets legal requirements.
Law enforcement: for limited law enforcement purposes permitted by law.
Coroners, medical examiners, and funeral directors: as needed to carry out their duties.
Organ and tissue donation: with organ procurement organizations.
Research: only when approved through a process that protects your privacy.
Workers' compensation: for workers' compensation claims.
Special government functions: for military, national security, and presidential protective services, as the law allows.
Uses that require your written permission
We will never do the following without your written authorization:
Use or share your information for marketing purposes.
Sell your information.
Share psychotherapy notes, if any exist.
Use your photo, story, or testimonial in our advertising or on our website or social media.
Any other use not described in this notice also requires your written permission. You may revoke an authorization at any time in writing. Revoking it stops future sharing but does not undo sharing already done.
Oregon law may give additional protection to certain information, such as genetic information, HIV test results, mental health records, and substance use treatment records. Where state law is more protective than federal law, we follow the stricter rule.
Your rights
To use any of these rights, contact us using the information at the end of this notice.
Get a copy of your records. You can ask to see or get a paper or electronic copy of your health record. We will respond within 30 days and may charge a reasonable, cost-based fee.
Ask us to correct your records. If you think information is incorrect or incomplete, you can ask us in writing to correct it. We may say no, but we will tell you why in writing within 60 days.
Ask for confidential communications. You can ask us to contact you in a specific way (for example, by cell phone only or email only) or at a different address. We will say yes to all reasonable requests.
Ask us to limit what we use or share. You can ask us not to use or share certain information. We are not required to agree, except in one case: if you pay for a service in full out of pocket, you can ask us not to share that information with your health plan, and we will agree unless the law requires us to share it.
Get a list of those we've shared information with. You can ask for a list of the times we shared your information in the past six years, who we shared it with, and why. This does not include sharing for treatment, payment, or operations, or sharing you authorized. One list per year is free.
Get a copy of this notice. You can ask for a paper copy at any time, even if you agreed to receive it electronically.
Choose someone to act for you. If you have given someone medical power of attorney, or someone is your legal guardian, that person can exercise your rights. We will confirm their authority first.
File a complaint. See "Complaints" below.
Your choices
For the following, you can tell us your preferences, and we will follow them:
Family, friends, and others involved in your care. We may share information with people you identify as involved in your care or payment. You can tell us who, or ask us not to share with anyone. If you are unable to tell us, for example, in an emergency, we may share information if we believe it is in your best interest.
Your name, pronouns, and communication. Tell us the name and pronouns you use and how you would like us to contact you. We will use them in person and in your communications with us.
Appointment reminders. We send reminders by text or email. You can choose your preferred method or opt out at any time.
Changes to this notice
We can change the terms of this notice, and the changes will apply to all information we have about you. The new notice will be available on request, in our office, and on our website.
